Transitional Aged Youth Survey

Regional community service providers would like to learn about the experiences of young people in the districts of Cochrane and Timiskaming moving from youth to adult mental health services.

This survey is being done to better understand the experiences of youth and to inform recommendations so youth in the region can better connect with the best mental health services and move easily from service to service.

The online survey is open to any youth* (14 to 21 years of age) who are currently receiving or have received a mental health service in the districts of Cochrane or Timiskaming and has or might be moving from youth to adult services.

The survey is open until December 17, 2021.

Those who complete the survey will have the opportunity to participate in a draw for a chance to win one of six $50 gift cards. An early bird draw will take place on December 8, 2021, the prize will be a $100 gift card. All entries completed by that date will qualify for the early bird draw and the main draw when the survey closes.

*Please Note: This survey is also open to the caregivers of youth (aged 14-21) who are receiving or have received a mental health service and who have or might be moving from youth to adult services.
