The Transition Home Program promotes a housing-first approach for youth, from the ages of 16-23, who are receiving services from the Agency and who require support with the transition to adulthood and acquisition of life skills.

These homes are overseen by on-site house parents who are approved foster parents in a home provided by North Eastern Ontario Family and Children’s Services (NEOFACS). The house parents and youth are supported by their resource worker and NEOFACS' multi-disciplinary team. Youth in the program live in a supportive housing environment.
