INTENSIVE SERVICE COORDINATION CASE MANAGER
Full-time Position in New Liskeard

As a member of the Child and Youth Mental Health team, the Intensive Service Coordination Case Manager works in collaboration with other service providers to provide comprehensive intensive service coordination functions for identified children/youth with multiple/complex special needs for all voluntary services funded by the Ministry of Children, Community and Social Services and the Ministry of Health and Long-Term Care in the rural, remote and urban communities. The successful candidate will develop, co-ordinate and manage the implementation of an integrated service plan to meet the identified needs of the child/youth and family.

JOB REQUIREMENTS (QUALIFICATIONS AND SKILLS):
• Graduate or Baccalaureate degree in Human Services. A College Diploma in Social Services may be considered along with other relevant university degrees or diplomas deemed suitable by the Executive Director;
• Experience working with children, youth and families is considered an asset;
• Excellent knowledge and skills in case management;
• Work effectively and positively within a multi-disciplinary environment and with service providers;
• Excellent communication, interpersonal and organizational skills;
• Must be willing and able to work flexible hours as required;
• Must be willing to travel as required;
• Bilingualism (English/French) is required;
• Candidates must possess a minimum G2 driver’s license at the time of application and be willing to obtain a valid G Class driver’s license as a condition of employment or already possess a valid G Class driver’s license and have access to their own vehicle.
